CMS releases its proposed rule for the coming year's Medicare Physician Fee Schedule every July. This year's release — covering 2027 — is expected within weeks, and it lands at a genuinely active moment for remote care policy.

The 2026 final rule split the conversion factor for the first time, applied a new efficiency adjustment to work RVUs, and introduced two RPM codes that didn't exist the year before. CMS also opened multiple comment solicitations — on APCM and prevention, on the annual wellness visit, on RPM and RTM valuation — that all but guarantee 2027 won't be a quiet year. And for the first time, a multi-year OIG audit of CCM payments is running in parallel with the rulemaking cycle, raising the compliance stakes for anyone billing these codes.
